Privacy Policy

The Professionals and staff of the Grand Bend Area Community Health centre are bound by law and ethics to safeguard your privacy and the confidentiality of your personal information.

This Includes:

Collecting only the information that may be necessary for your care;
Keeping accurate and up-to-date record
Safeguarding the medical records in our possession;
Sharing information with our heath-care providers and organizations on a "need to know" basis where required for your health care;
Disclosing information to third parties only with your express written consent, or when necessary for legal reasons; and
Retaining/destroying records in accordance with the law

Your request for care from Grand Bend Area Community Health Centre implies consent for our collection, use and disclosure of your personal information for purposes related to your care. As noted above, other purposes require your express consent.

You have the right to see and/or make corrections to your records. Please speak to our Health Information Custodian, Heather Klopp to arrange a mutually convenient time. If you need a copy of parts of your record, please speak to the receptionist who will ask the Health Provider to provide it for you or another health care provider. We will also provide a transfer copy of your records to another physician with your written consent. There may be a fee for these services - please ask the receptionist.If you would like to discuss our privacy policy in more detail, or have specific questions or complaints about how your information is handled, please speak to your Health Providers or our Medical Services Coordinator.
