We offer full time occupational therapy services by Kate Mason and Shawna Parmar, Registered Occupational Therapists within the Aging at Home Program.
These services are currently available to patients of the Grand Bend Area Community Health Centre as well as other seniors within the catchment area. Referrals are received from physicians, nurse practitioners, other health care professionals, caregivers, or the client themselves.
What is Occupational Therapy?
Occupational therapy addresses participation in everyday living by goal setting to enable individuals to perform the occupations that they find meaningful. By enabling individuals to participate in activities they need and want to complete, health and wellbeing is restored and/or maintained.
What is an Occupational Therapist?
A professional who works with individuals to assist them in developing, improving, sustaining, or restoring their highest possible level of independence in their desired daily activities. Occupational therapists look at areas including personal care, productivity and leisure and address physical, cognitive and emotional concerns.
What does an Occupational Therapist do?
Our occupational therapists start with a visit to your home in which they will ask you to describe your condition, your history, lifestyle and goals related to activities of daily living. An occupational therapist may help with:
Physical Concerns – fall prevention, home safety, device use, safely returning home from a hospital visit, education to regain/maintain independence with daily tasks
Emotional Concerns – provide information and form connections to social activities/groups in the community, help with coping strategies
Cognitive Concerns – help to understand and live with memory changes/confusion, provide strategies
Referrals to other professionals/programs (i.e. social work, dietician, diabetes program, CCAC, in-home support services, community services, exercise programs, etc.)
Assessments for Assistive Devices Program (a government program to provide financial assistance for required mobility devices such as rollator walkers, wheelchairs, etc.)
