Better Breathing Team
The Better Breathing Team is a mobile team of healthcare professionals whose mandate is to serve the population of clients living with chronic lung impairments, primarily Chronic Obstructive Pulmonary Disease (COPD).
As part of the Grand Bend Area Community Health Centre’s larger “Aging at Home” Program, the Better Breathing Team works to assess patient needs, perform diagnostic tests, and facilitate pulmonary rehabilitation programs. Referrals may be made to other health care professionals as needed.
How will the Better Breathing Team work in the community?
This mobile team will provide care throughout the various communities in its catchment area: Dashwood, Exeter, Grand Bend, Hensall, Petrolia, Port Franks, Oil City, Oil Springs, Thedford, and Zurich. Team members will address immediate respiratory concerns, help to optimize the use of respiratory medications, and educate patients on how to manage their symptoms at home. They will also address other prevalent needs that require support from other community referrals and resources.
Who is on the team?
The Better Breathing Team is a multidisciplinary team of regulated Allied Health professionals and includes a Kinesiologist, an Occupational Therapist, a Physiotherapist, a Respiratory Therapist, and a Social Worker.
The Better Breathing Team receives referrals from various sources within the catchment area. The Team will work collaboratively with community programs, other community health centres, hospitals, family health teams, and independent physicians. The Team offers their support and expertise to these referral sources and develops educational resources that are available for community use.
